Copays & Deductibles: Payment is due at the time of service. We require a valid credit card to be kept on file in our secure, PCI-compliant system.
Self-Pay: If you do not have insurance or prefer not to use it, we offer self-pay rates. Please contact our office for the current fee schedule.

Please note that insurance plans generally do not cover the preparation of forms, reports, letters, or other administrative documents. These services require provider review, documentation time, and clinical judgment. Fees for these services are the responsibility of the patient.

Cancellation Policy & Mutual Respect for Time
We strive to provide timely care to all our patients. When an appointment is missed or canceled without sufficient notice, it prevents another patient from receiving the help they might urgently need. Therefore, we kindly ask for at least 24 hours advance notice if you need to reschedule.
Missed appointments (No-Shows) or cancellations within this 24-hour window will incur an administrative fee of $150.00 to account for the provider's reserved time.
* Please note that this administrative fee is NOT covered by insurance plans and is the sole responsibility of the patient.
